Your care and recovery is our priority. That’s why our care coordination team works to organize care for the patients that are hospitalized at GRMC. Our role in the hospital care team is to look for any needs you may have, as a patient, as you leave the hospital.
We work with the patients and their families to develop an appropriate plan for their discharge from GRMC to ensure a smooth transition from one level of care to another. This could include setting up home healthcare services, arranging for needed medical equipment, or when necessary, helping patients get to a skilled nursing facility or an acute rehabilitation unit.
Our team continues to work with patients after they leave the hospital in many cases. We contact patients within a few days after their discharge to see how they are doing and if they have any questions about their plan of care for after the hospital stay. We can review and reinforce discharge instructions or help patients with anything they didn’t understand in their instructions. We also work closely with the patients’ healthcare providers if the patient is having any recurring or new issues.
